Deadline Approaching: Survey to Assess Churches’ Facility Use by Community Members

Partners in Health and Wholeness would like to learn about the use of your congregation’s facilities by outside groups and individuals through a Faith Community Facility Use Agreement Survey. Findings from this survey will help our staff and community partners – i.e., Voices into Action, Faithful Families Eating Smart and Moving More, and the NC Division of Public Health – develop resources and trainings on how congregations can use their facilities to promote physical activity.
