Apply Today for a PHW Mini-Grant

February 13, 2018 by Katie Withers, NC State MSW Intern

There are less than two weeks remaining to apply for a Partners in Health and Wholeness (PHW) Mini-Grant.

Congregations that are members of the PHW Collaborative are able to request up to $1,000 for a mini-grant. Eligible congregations must have been accepted into the PHW Collaborative or renewed their membership in the past 12 months. They also cannot have received a mini-grant in the summer 2017 grant cycle. We encourage churches to ask for a reasonable amount to fulfill their health plans outlined in the mini-grant application.

The mini-grant must be used to support health and wellness activities outlined and approved in your mini-grant application. Mini-grants should be used for projects that engage congregants and fulfill a health need in the church or in the surrounding community. Priority is given to projects that address at least one of PHW’s four cornerstones: healthy eating, physical activity, tobacco prevention and cessation, and mental health. PHW has previously funded initiatives such as exercise classes, supplies for health screenings, starting/expanding a community garden, hosting support groups, health education classes, and purchase of tobacco cessation signs. For answers to additional questions, please visit our Frequently Asked Questions page or reach out to the regional coordinator in your area (their contact information can be found here).

The best way to apply is online by clicking here. We are currently updating the printable version of the form, but if you would like to submit a hard copy of the application, please mail it to:

Partners in Health and Wholeness
27 Horne St, Raleigh, NC 27607

Make sure it is postmarked by the deadline, February 28th, 2018. It is important to keep your own copies of all items in case they are not received by PHW. We highly recommend following up with Partners in Health and Wholeness to confirm that your application was successfully received. If you are unable to use any of the submission options above, please contact PHW by calling (919)-828-6501 or emailing PHWinfo@ncchurches.org and we will do our best to assist you.

She is excited to continue helping individuals who are disenfranchised and marginalized. Katie is currently attending North Carolina State University in pursuit of a Masters of Social Work degree.
